Who We Are
CASE MANAGER (CM):
We will be directly involved in the mental health of the clients and will provided assistance and coordination of access to community resources. The Case Manager provides follow up to the clients in the area of mental health. The case manager helps individuals, and their families identify the resources needed to access the medical, financial, and educational services that are vital in recovering mental health conditions.
What We Do
01.
Coordination of care with service providers. Monitor medication treatment. Assistance to select medical providers and specialist.
02.
Asistance to Apply for Low-income Housing or Plan 8. Plan * Recertifications and locations transfers.
03.
Apply for assistant in living or home health services (HHA Service).
04.
We Educate seniors with daily living activities and obtain or coordinate special and regular transportation services.
05.
Economic assistance (assistance to obtain food donation, personal and hygiene items donation, households’ toiletries donations, Clothing donation, and furniture donation).
06.
Assistance with legal documentation (residence, citizenship, pasport and identification.)

Case Manager serves as means to achieve the welfare and autonomy of the client. It is Highlighted through communications, education, the identification of resources and the facilitation of services.
The Case Manager helps identify health providers for better client welfare. The Case Manager service are best offered directly between the case manager, the client, and the staff. Case Manager will assist the client in order to improve the quality of life.
